Chicago: Driver Documentation Examples

Curb is thrilled to welcome you to the team in Chicago! In order to have a smooth onboarding process, we want to ensure you have all the necessary information.

Please refer to the images below for examples of acceptable documentation. These documents are crucial for completing your onboarding and ensuring you're eligible to receive trip offers from Curb in Chicago.  

Taking Photos of Your Documents:

• Capture the image from above and ensure the entire document is in the frame.
• Include all four corners of the document.
• Place the document on a flat surface with a clean, white background to enhance visibility.
• Ensure your camera is focused before taking the photo and hold the camera steady to avoid any blurriness.
• Review the photos to ensure all texts and details are clear and legible. 

Taxi Chauffeur License

Livery Chauffeur License

Public Chauffeurs for both FHV and Taxi are required to have a Public Chauffeur License issued by the City of Chicago.

The City of Chicago maintains the Chicago Data Portal in which the status for both the Chauffeur License as well as the Medallion can be tracked. The portal must maintain an “Active” status  for both the Chauffeur License and Medallion number in order to complete onboarding and enable drivers to go online. Vehicles and drivers that are not compliant with items such as vehicle inspection, insurance, and payments will be suspended by the Department of Business Affairs and Consumer Protection (BACP).

Note that the documents can differ since there are several insurance companies. Please ensure that the insurance document you submit contains the VIN number, plan number and expiration dates.

Taxi Vehicle Identifiers 

The Illinois Secretary of State issues license plates that are numbered 1 to 6999 and contain the suffix ”TX”.

FHV Vehicle Identifiers 

The Illinois Secretary of State issues license plates that are numbered 7000 to 9999 or 70000 to 73000 and contain the suffix “LY”
